How Does Something Like This Happen?

How does something like this happen? How, in the 21st Century, in 2013, does this sort of thing still happen? How does a card like this go from concept, to design, to proof, to print, to die-cut, to assembly, to pack-out, into my hands and then into a package headed to its new owner before ANYONE speaks up about it?

Now, I know I’m not perfect. I make just about as many mistakes as the next person. The thing is, I’m just one person. I don’t have an entire army of people looking over my work before it gets approved for public consumption (somedays I wish I did). So, how, in this day and age, does something like this happen to a hit in a high-end product?

    Not an error card I would want in a higher end product. like the sought after Jose Fernandez that was on the Mariners! They need to do something around there so that these don’t happen

    More proof that Topps really doesn’t give a sh*t about what they produce – just that they flood the hell out of the market to get our money.
